Many of you have already enrolled in the Osage Nation's new tribal Elder health benefit — the Group Medicare Supplement insurance Plan F, underwritten by United American Insurance Company and paid for by the Osage Nation. ➊
OPEN ENROLLMENT: October 15 - November 30, 2015. ➋
Your coverage will be effective January 1, 2016.
+ Instead of the limited health benefit you now have, you will have the most complete Medicare Supplement plan available — all at no cost to you.
+ You can go to any provider, hospital, or clinic that accepts Medicare.
+ Eligible spouse coverage is also available. ➌
! You MUST be enrolled in Medicare Part A and B to participate in this plan.

➌ Monetary value for the Medicare Supplement insurance Plan F for Elders is $193 per month, or $2,316 annually and paid for by the Osage Nation. If your Non-Osage Medicare eligible Spouse chooses to enroll, you are responsible for paying the $193 monthly premium through either direct bill or bank draft.
